• Participate in the development, implementation, and evaluation of individualized support plans. 

• Provide person-centred support to promote independence, life skills development, community inclusion, and overall well-being. 

• Assist with activities of daily living, personal care, medication administration, and specialized care as required. 

• Support social, recreational, educational, and vocational activities 

• Respond to emergencies, behavioural challenges, and health and safety concerns. 

• Monitor, document, and report on individual progress and program outcomes. 

• Coordinate with families, professionals, and community resources to support individual goals. 

• Provide transportation, household support, and other related duties as assigned.
