Responsibilities
Participate in the development, implementation, and evaluation of individualized support plans.
Provide person-centred support to promote independence, life skills development, community inclusion, and overall well-being.
Assist with activities of daily living, personal care, medication administration, and specialized care as required.
Support social, recreational, educational, and vocational activities
Respond to emergencies, behavioural challenges, and health and safety concerns.
Monitor, document, and report on individual progress and program outcomes.
Coordinate with families, professionals, and community resources to support individual goals.
Provide transportation, household support, and other related duties as assigned.
